It's basically from people over exerting themselves to an extreme degree due to not being ready for the intensity of a work out.  This can voluntarily happen to people and it does all the time actually.  It used to be super rare but in the recent years there has been a rash of it due to the fast rise of crossfit.  It's not a one size fits all thing since in order to get it you have to ignore your body to get it and not everybody has the same pain threshold to extreme over exertion.  What looks like just a  normal case of a tough workout causing somebody to struggle to a coach/trainer can easily escalate to this kind of case due to it being up to the athlete to also know it's limits. Then you add in the competitive/"push yourself bro"  environment and it's easy to see where this can happen even under reasonable circumstances.  

 

If it was easy to see this happening, you'd actually see it happening more often. Google it and you'll find Oregon and Iowa 6 years ago.

I did see that, I was thinking more for college football where you have a staff who's job it is is to make sure things like it don't happen.

As someone who hasn't done crossfit but know a lot of people who do the answer yes the gyms typically have a couple trainers on staff at all times. I'm more of a running and recently yoga kind of guy personally...

His tenure there has been nothing short of a **** show and this doesn't help. He sounds whiny as hell. His strength coach got suspended for a month. His OC gets a DUI. His WR coach is in the car and leaves the school  after the incident.  I feel like this is just the beginning. If WT doesn't win quick there, he could be out quick.
